Some companies, such as the Better Business Bureau, allow its users to submit complaint forms regarding other organizations. Meanwhile, other organizations will collect these complaints about themselves in order to manage employee actions or improve their business operations.

A complaint form template is a pre-built, standardized grievance form that can be easily replicated or repurposed for different uses.

By using a template, you can easily collect complaints from employees and customers without building an entirely new form from scratch. This can save you time, money, and energy, as well as ensure that you collect all crucial information upfront.

To build a complaint form, go to the Reform builder and choose a starting template or build a new form from scratch. If you begin with a template, you can easily modify the fields by selecting the gear icon next to them. To add new fields, text, or images, choose “add block” and select your preferred option. You can even customize the images and add a logo if you want to.

A basic complaint form should include the following information:

  • Name
  • Contact information
  • Specifics about the user’s relationship with the company (e.g., department of an employee or account manager of a client)
  • Date of the incident
  • Approximate time of the incident
  • Location of the incident
  • Description of the incident
  • A list of others involved or witnesses to the incident

Yes! Reform allows you to set up email notifications so you can be alerted every time a complaint is submitted. These emails can either go to you (the form creator) or a designated member of your organization. If you prefer not to receive notifications, you can also disable this feature.

Complaint submissions will automatically be stored within your Reform account so you can easily view all submissions or export them in a CSV file. We also have a Zapier integration that allows you to send submissions to your chosen platform.

Building your complaint form with Reform is completely free if you only need to include basic fields. For most standard complaint forms, a free Reform plan will allow you to collect all the necessary information.

We suggest embedding your form on a page of your website that can be accessed within the footer navigation, if you want to collect customer complaints. For employee complaints, try embedding the form on your HR platform or Intranet and making it easily accessible.
